Job description
Overview

The Community Psychiatric Nurse will be a member of the Older Adult Community Mental Health team, working closely with the multi-disciplinary team.

Responsibilities
  • Maintain clinical caseloads and conduct assessments of highly complex problems which require analysis, interpretation and comparison of a range of solutions for service users who are experiencing severe and complex mental health difficulties, both organic and functional.
  • Take responsibility for allocated caseloads to ensure assessments of health and social needs in line with their code of practice.
  • Formulate, plan and evaluate care, including regular reviews and updates of risk assessments, documentation and care planning.
  • Act as a care co-ordinator and advocate for the service user when appropriate and promote independence through the recovery process.
Requirements / Qualifications

The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
