An exciting opportunity for a Community Psychiatric Nurse with the NHS in Wales, working within the Older Adult Community Mental health team. The role involves managing clinical caseloads, conducting assessments, and formulating care plans for service users experiencing complex mental health issues. The position offers extensive training opportunities and promotes work-life balance.
The Community Psychiatric Nurse will be a member of the Older Adult Community Mental health team, working closely with the multi- disciplinary team. Responsibilities include the maintaining of clinical caseloads and conducting assessments of highly complex problems which require analysis, interpretation and comparison of a range of solutions for service users who are experiencing severe and complex mental health difficulties, both organic and functional.
The Community Psychiatric Nurse will work within their code of practice to take responsibility for their allocated caseload to ensure assessments of health and social needs. This will include formulation, planning and evaluation of care which include regular reviews and update of risk assessments documentation and care planning. They will be a care co-coordinator and advocate for the service user at times and promote independence through the recovery process.
The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
