Job summary

Devon Partnership NHS Trust is seeking a Community Psychiatric Nurse (Band 6) to provide community-based mental health support. This NHSP bank role offers flexible shifts across Exeter and surrounding areas, including Torquay, Ivybridge and Barnstaple, with a pay rate of £22.99 per hour.

The post involves robust patient assessments, care planning, MDT involvement and liaison with multidisciplinary teams, enabling professional autonomy and diverse clinical experience.

Qualifications

  • Minimum of 6 months paid experience as a Community Psychiatric Nurse in a UK hospital environment within the last 3 years.
  • Comply with NMC professional registration and keep up to date with nursing practices.
  • Full UK driving licence and own vehicle.

Responsibilities

  • Provide robust assessment of patients (telephone or face-to-face) referred for mental health needs.
  • Identify appropriate care pathways and ensure least restrictive options.
  • Signpost to primary and tertiary care providers as needed.
  • Raise concerns to senior staff for support and management.
  • Complete documentation and plan care for patients.
  • Present cases to the Multi-Disciplinary Team for care pathway discussion.
  • Liaise with nursing, occupational therapy, medical, psychology and admin teams.

Shift Patterns: Monday to Friday 9-5

  • Providing robust assessment of patients (by telephone or face-to-face) who have been referred due to presenting with mental health difficulties.
  • Identifying appropriate care pathways and ensuring the least restrictive option to meet the needs of the patient and ensuring their wellbeing.
  • Signposting to appropriate care providers (including primary and tertiary care).
  • Raising concerns to senior staff for support and management.
  • Completing documentation and planning care for patients.
