We are currently recruiting for a skilled Band 6 Community Psychiatric Nurse (CPN) to join the Psychiatric Liaison Team at the Long Fox Unit, Weston General Hospital. This is a front-line mental health role where you’ll work as part of a dynamic team assessing and supporting patients in acute hospital settings who present with mental health needs.
You will be expected to conduct autonomous assessments and provide swift mental health interventions, working closely with acute hospital teams, emergency departments, and wider community services. Key Responsibilities:

• Carry out autonomous, front-line psychiatric assessments of patients presenting in crisis or with acute mental health needs.
• Liaise with A&E, inpatient wards, and community services to ensure appropriate and timely care pathways.
• Formulate risk assessments, care plans, and make clinical decisions independently under pressure.
• Contribute to discharge planning and onward referrals to community or specialist services.
• Maintain accurate documentation and follow trust policies and clinical governance protocols.

Essential Skills & Experience:

• Registered Mental Health Nurse (NMC).
• Experience in liaison psychiatry or acute mental health settings.
• Proven ability to work independently in high-pressure environments.
• Strong clinical assessment and risk management skills.
• Understanding of mental health legislation and safeguarding.
