Qualifications

  • A minimum of 6 months paid experience as a Community Psychiatric Nurse in a UK hospital environment within the last 3 years.
  • Comply with NMC Professional Registration and keep up to date with current nursing practices.
  • Full UK driving licence and own vehicle.

Overview

Responsibilities
  • Providing robust assessment of patients (by telephone or face-to-face) who have been referred to the CMHT due to presenting with mental health difficulties.
  • Identifying appropriate care pathways and ensuring the least restrictive option to meet their needs and ensuring their wellbeing.
  • Signposting to appropriate care providers (including primary and tertiary care). Attending daily Red Board meetings.
  • Raising concerns to senior staff for support and management. Completing documentation and planning care for patients.
  • Presenting cases to Multi-Disciplinary Team for discussion around care pathway.
  • Liaison with colleagues nursing, occupational therapy, medics, psychology and admin.
