An opportunity has arisen for a community mental health nurse to join our dynamic team working within the Dementia and Older Persons Mental Health team supporting older persons with Dementia and Mental Health in the community. Working hours are 9am - 5pm, Monday to Friday. This post will be part of our multidisciplinary community team, and the successful applicant will work within a supported team to deliver interventions to individuals with complex needs. Our goal is to help people remain safe and well at home and maintain important relationships.
The role of the Community Nurse involves working in partnership with service users within the designated care group, developing care and recovery plans to enable them to lead fulfilling lives. The nurse will demonstrate understanding of clinical interventions relevant to the care group, including education, care planning, and discharge planning, working closely with carers, families, and partner agencies. The post holder will always promote the safety and well-being of service users and their families or carers. Responsibilities include assessing, planning, and implementing care, providing specialist nursing advice, and carrying out specialist procedures.
The post holder will be an autonomous practitioner responsible for a shared caseload, undertaking assessments of individuals and their families with complex health presentations, including high-risk cases. They will develop care plans collaboratively, offer specialist assessments and advice, and manage ongoing risks in partnership with service users and carers. The role includes contributing to the development of the Trust's nursing strategy, ensuring evidence-based practice aligned with NICE guidelines and Trust policies, supervising junior staff and trainees, and acting within the NMC code of conduct.
Travel across sites is essential for this role.

We are committed to safeguarding and expect all staff to be familiar with relevant policies and undertake necessary training. Salary will be at the bottom of the relevant range unless NHS experience justifies a higher starting salary.
