Job description
Overview

An opportunity has arisen to work as a Physiotherapist within the South Community Therapy Team on Ogden Court, Community Unit, Wymondham. The unit provides multidisciplinary assessment, treatment and discharge planning for patients who require therapy interventions before returning home/to placement. The Ogden team have close links to the team at our other bedded unit at Foxley (Dereham) and the Community based Therapy Team.

You will have the opportunity to develop your clinical knowledge and skills with support from the Clinical Lead Physiotherapists. You will be working closely with other professionals in the team and will acquire holistic assessment skills to support your core Occupational therapy knowledge.

You will deliver high quality physiotherapeutic interventions and health care to patients within a variety of community settings, which may include in-reaching on to the in-patient unit as needed. To provide proactive, holistic case management and rehabilitation whilst working with patients to help them set realistic goals. Utilising health coaching skills to actively engage with patient and their family to ensure that expectations are appropriately managed.

Responsibilities
  • To work on Ogden Court ward as part of the MDT
  • To assess and describe treatment packages for patients with long term conditions, the frail & elderly, palliative care and rehabilitation needs for example, to achieve quality of life and independence where possible.
  • To work within the integrated team to facilitate early discharge from hospital as part of the Discharge to Assess Pathway.
  • To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
  • To supporting students, band 5s, and unregistered staff through supervision and informal discussion
Development & Support

There is an in-service training programme to promote further learning, and there is regular clinical supervision and an annual appraisal to develop you in your post. Physiotherapy students are taken on a regular basis from the University of East Anglia. You will be able to develop your leadership skills by supervising junior staff and students in your role as a Senior Physiotherapist.
