An exciting opportunity has arisen in South for a physiotherapist to join our community therapy service in Thetford. You will work with and support a team of friendly, approachable physiotherapists, occupational therapists, and community assistant practitioners. The post will be based at Tree Tops Nursery, Thetford, and will require travelling to see patients in their own homes or in care homes in the surrounding villages.
You will deliver high-quality physiotherapeutic interventions and healthcare to patients within various community settings, including in-reaching on to the in-patient unit as needed. Your role will include providing proactive, holistic case management and rehabilitation, working with patients to set realistic goals. Utilizing health coaching skills, you will actively engage with patients and their families to ensure expectations are appropriately managed.
You will assess and prescribe exercise programmes for patients with conditions such as frail elderly, palliative care, and rehabilitation needs, aiming to promote quality of life and independence where possible.
You will work with the integrated team to facilitate early discharge from hospital and prevent unnecessary admissions. Collaboration with healthcare professionals, statutory, and non-statutory agencies will be essential to provide a seamless, integrated service to our service users.
You will also provide advice, support, and supervision to junior staff, therapy assistants, and students.
