Humber Community Services NHS Trust is seeking a thoughtful physiotherapist to join the Urgent Community Response and Intermediate Care Services in Scarborough. You will assess and treat patients in homes and care homes across Scarborough and Ryedale Locality, with support from senior physiotherapists.

The role requires HCPC registration (or near qualification) and a commitment to CPD, reflective practice, and multi-disciplinary teamwork.

Qualifications

Responsibilities

• Provide line management, supervision, support and training to junior staff and students

Job description

We are looking for a thoughtful, proactive and enthusiastic individual to join the friendly team within our Urgent Community Response and Intermediate Care Services Team in Scarborough. The role would suit either a newly qualified Physiotherapist or a Physiotherapist looking to gain Community experience. As a community Physiotherapist you will be working with the Urgent Community Response, Intermediate Care Team, assessing and treating patients referred by all our health and social care stakeholders, providing a high standard of care and physiotherapy services to patients with diverse and sometimes complex conditions in their own homes and care homes. Because the patients are community based it is essential to have a car and driving licence for the UK. You will be responsible for providing specialised physiotherapy assessment, treatment and advice to an independent caseload, working closely with colleagues in this multi-disciplinary team and supported by senior Physiotherapists. This post works across Scarborough and Ryedale Locality across Humber Community Services. Within this role you will:

• Perform comprehensive physiotherapy assessment of patients with diverse presentations, using clinical reasoning skills and manual assessment
• Formulate and deliver specialist individual physiotherapy treatment programmes based on a sound knowledge of evidence-based practice
• Formulate accurate prognoses and recommend best course of intervention
• Evaluate patient progress, reassess and alter treatment programmes if required

• Diploma or Degree in Physiotherapy (or in final year of programme)
• Health and Care Professionals Council (HCPC) registration if qualified, Evidence of relevant CPD - could include completion of short courses or post-graduate study.
• Completion of Physiotherapy preceptorship, Knowledge of the specific service area, team and role evidenced within the supporting information of application to vacancy
• Experience of Community Physiotherapy in prior NHS employment or student placement.
• Demonstrates knowledge and understanding of reflective practice, Experience of managing a caseload of patients including triage and prioritisation of referrals and daily work, Evidence of how you will efficiently transport self and equipment to/from multiple venues over a large geographical area during each shift in the UK
