Job summary

Sheffield Health Partnership University Nhs Foundation Trust is seeking a Physical Health Support Worker to work with the South Community Mental Health Team in East Glade. The role focuses on promoting recovery, wellbeing, and addressing physical health needs within the community.

You will work autonomously with a caseload, under supervision, delivering care aligned to individual care plans and NICE guidelines, including physical health assessments and health promotion.

Qualifications

• Excellent written and verbal communication skills.

Responsibilities

• Undertake routine physical health interventions and monitoring (venepuncture, ECGs, BP, height/weight).
• Deliver health promotion and wellbeing advice (nutrition, activity, smoking cessation).
• Arrange follow-ups per NICE guidelines and clinical need.
• Maintain stock and signs, dispose of sharps and waste, calibrate equipment.
• Support care plans and engage service users in relevant services.
