Job summary
Mountbatten Isle of Wight is seeking a compassionate Nursing Associate to join our Community Services Team on a 12-month fixed-term contract. You will help deliver 24/7 palliative and end-of-life care across community, residential and nursing settings, working within a multidisciplinary team to support patients and families.
Holders of the Nursing Associate Foundation Degree and current NMC registration are essential, with commitment to ongoing professional development and flexible working hours
Qualifications
- Registered Nursing Associate on the NMC Register.
- Nursing Associate Foundation Degree qualification.
- Care Certificate Qualification/NVQ Level 3 in Health and Social Care.
- Clinically up to date in general nursing care.
Responsibilities
- Develop and maintain communications with patients, carers, other professionals and volunteers.
- Work closely with multidisciplinary teams to ensure provision of individualised care.
- Understand the role of Nursing Associate within Mountbatten and MDT, contributing to service development.
- Work with registered nursing staff in assessment, planning and evaluation of patient-centred care.
- Seek support from registered nurses and MDT when required.
- Provide telephone advice for patients, carers and other professionals from the Community Co-ordination centre.
- Give and receive accurate reports and changes in patients' condition.
- Undertake joint assessments with other professionals and liaise regarding ongoing care.
