• Coordinate with primary and secondary care teams to plan and deliver care.
• Maintain accurate records and collaborate with City Hospice teams to ensure continuity of care.
• Uphold professional standards and participate in ongoing education and supervision.
• Support and promote holistic day-care activities within the hospice setting.
