Community Palliative Care Nurse / Community Palliative Care Paramedic (Development Opportunity)
Contract: Permanent / 37.5 hours per week
Hours of work: The post holder will be expected to work as part of a 7-day service and may work an 8am-8pm rota as part of service development in line with the NWL ICB New Model of Care.

Job summary
The clinical nurse specialist or clinical paramedic specialist acts as a key worker for people living with cancer and other life-limiting illnesses. They conduct holistic needs assessments, develop care plans, and deliver high standards of personalized care, providing physical and emotional support, coordinating care services, and advising on clinical and practical issues, leading to positive patient outcomes.
Leadership
This role offers development into a Clinical Nurse Specialist or Community Paramedic Specialist (Palliative). You will work closely with our service lead team, supported to develop necessary skills and competencies.
As part of the Community Specialist Palliative Care Team, you will provide specialist advice and care to patients, families, carers, and other professionals within North Brent.
You will manage your own clinical caseload, support individuals with non-curative diagnoses, and assist primary care services with complex management, acting as a coordinator for palliative care support across providers.
You will safeguard patient interests, practice according to professional standards (NMC or HCPC), and participate in on-call duties over a 7-day service.
Clinical Care
You will be responsible for a caseload within the catchment area, supporting individualized care, communicating effectively, liaising with professionals, and ensuring confidentiality. You will promote holistic health assessments, advance communication, and provide bereavement support, working collaboratively with multidisciplinary teams.
Managerial
You will work with the Community Team Leader to ensure effective team functioning, manage your caseload, maintain accurate records, participate in team meetings, audits, and quality initiatives, and ensure high standards of care and resource use.
Education/Training and Quality
You will maintain professional competence, participate in training and education, contribute to audits, and stay updated with developments in palliative care nursing.
