Description of Role:
Working as full members of the CDP Team, Community OT will primarily be responsible for providing a person-centred service through specialist clinical assessment within the context of Social Care and Health policy and legislation.
The post involves managing a caseload and lone working in a variety of settings. The emphasis of the work is a combination of OT and specialist moving and handling clinical skills, knowledge with responsibilities involving highly skilled and specialised work. The tasks will include: Undertaking specialist moving and handling assessments and reviews within hospital and community settings. This will be where the person is primarily in receipt of D/H care and/or where double handed care is requested, but not exclusive to this.
Prescribing and implementing effective strategies to achieve person centred and cost effective outcomes for individuals.
Managing a caseload including highly complex cases within the legal framework.
Acting as an autonomous practitioner evidencing high levels of problem solving, decision making and independent judgement.
