Job description

JustOT are looking to recruit a band 6 Community Occupational Therapist for a locum position with our client in Essex.

The pay for this position is £21-£24 per hour, and we are looking for someone who can start as soon as possible for an ongoing duration.

Job Description:
You will be responsible for undertaking assessment and intervention in the community and care homes for patients under the Discharge to Assess (D2A) pathways. Assessing patient's therapy and care needs, provide intervention to promote safety and independence , and aim to reduce the risk of re-admission or harm and to help support people onto the best pathway for their needs. You will coordinate services by undertaking a person-centred assessment; developing, implementing and evaluating care and therapy plans, assessing equipment needs, supporting discharge planning and working on rehabilitation goals to promote function and independence. Post holder will work autonomously within professional guidelines and the overall framework of the service's policies and procedures. Duties will include assessing for and health or social care packages to meet the patient's needs. All these skills are competency led, and appropriate training will be provided where needed.

Successful candidates must hold a degree in occupational therapy and be HCPC registered.
