Leeds Community Healthcare is looking for Community Nursing Assistants to join their Neighbourhood Night Service. This role involves providing critical nursing intervention to patients in their homes and supporting a multidisciplinary team to ensure quality care. The position requires working overnight shifts with a minimum contract of 18 hours per week, offering valuable experience in community healthcare.

The Neighbourhood Night Service operates from 20:45 to 07:15, providing care across Leeds to patients requiring nursing intervention in their own homes. A key aim of the service is to prevent unnecessary hospital admissions and to ensure that palliative patients remain in their own homes where this is their preference. The service is part of a multi-disciplinary team and ensures 24/7 nursing care for patients in the community.
We will support you with a tailored induction programme and will encourage and facilitate ongoing development, including the opportunity to develop and learn new skills. We strongly recommend getting in touch to discuss the role.
You will support the Registered Nurses, responding to intervention call outs overnight or working alongside a colleague to visit patients for planned interventions. You will also support the Virtual Ward (Frailty), providing coordinated rapid care to people in their homes experiencing an acute medical episode.
It is expected that you are available to work on a rota over 7 nights per week, 365 days per year. Rota schedules will be issued with a minimum of 4 weeks' notice. Your hours of work will be from 21:45 to 07:15. The contract is for a minimum of 18 hours (2 nights) up to a maximum of 36 hours (4 nights). Hours can be discussed prior to application or at interview as required.
