Job summary

NHS Cornwall and Isles of Scilly Trust is seeking caring and motivated Community Nurses to join a friendly team. You will work across Liskeard, Looe and surrounding areas, visiting patients in their homes and collaborating with GP practices and MDTs.

Role requires autonomous assess/plan/implement/evaluate care, with induction and supervision provided. Travel to patient homes is required; access to own transport advised due to limited public transport in Cornwall.

Qualifications

  • Registered Nurse (Adult)
  • Evidence of recent relevant professional development
  • Mentor for student university placements and mentoring qualification or willingness to work towards it
  • Current Professional Registration - NMC; post-registration diploma or degree in nursing; IT skills incl. Word/Excel/email/internet/recording systems
  • Ability to provide skilled, evidence-based nursing care to a wide range of patients
  • Ability to support patients and carers under stress
  • Awareness of policies including lone worker and manual handling
  • NHS community and mental health provider Trust based in Cornwall and Isles of Scilly; delivers community and hospital-based care and supports dementia/learning disability.

Job description

We are looking for caring, motivated and dynamic Community Nurses to join our friendly, supportive team. This is a great opportunity for someone who is confident working autonomously while still valuing strong team collaboration.You will be part of the Liskeard-Looe Community Nursing Team, working across the area, covering Liskeard, Looe and the surrounding areas. Our team works with local GP practices (Oaktree, Rosedean and Oldbridge Surgeries) as well as health and social care colleagues, therapists and voluntary sector partners. We're looking for someone who brings enthusiasm for community nursing, is committed to person-centred care and is keen to contribute to innovative ways of meeting the changing needs of our community.We welcome applications from nurses who are proactive, adaptable and confident in assessing and managing care in patients' homes. An interest in working closely with patients, carers and wider MDT partners is essential, along with a commitment to continuous professional development. The role involves regular travel to patient homes and community sites, along with the transportation of essential clinical equipment. Given the limited public transport options in Cornwall, candidates must have access to appropriate transport that can be used for work duties.Main duties of the job Shift patterns are 08:00-16:00 and 14:00-22:00. Following an initial induction and ongoing clinical supervision, you will be responsible for: Assessment, planning, implementation and evaluation of care, without direct supervision, to patients, carers and relatives within their home environment. The aim is to enable them to enjoy maximum independence and optimum quality of life.To work in partnership with patients and carers to offer support and advice and promote informed choice.To work autonomously, providing effective, evidence-based care in line with competency and training record and organisational policies, procedures and guidelines.To support all locality teams by working in a flexible way.   • Registered Nurse (Adult)
  • Evidence of recent relevant professional development
  • To act as a mentor for student university placements. Having the mentoring qualification or being willing to work towards this qualification and attending mentorship updates as required.
  • Current Professional Registration - NMC, Hold a post-registration diploma or be working towards a degree, IT skills to include use of Word, Excel, email, internet, recording systems
  • The ability to provide skilled, evidence based nursing care to a wide range of patients
  • Ability to deal with patients and carers who are under stress.
  • Awareness of and adherence to policies including lone worker and manual handling policies
  • Current knowledge of developments in nursing and healthcare in general. We're an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people's physical and mental health. We also provide specialist support to people with dementia or a learning disability.
