As a Community Nurse, you would be supported to develop into an autonomous practitioner whilst nursing patients with multiple complex health care needs in their own home. District nursing is a highly specialised area of practice recognised by the Queens Nurses Institute and NMC as a specialist role.
As a Community Staff Nurse, you will work within a neighbourhood team of District Nurse Team Leaders, Community Staff Nurses, Therapists, Assistant Practitioners, Health Care Assistants, and phlebotomists to meet the needs of the complex patient in the community and avoid admissions to the acute sector as far as practicable. This includes:
We nurse people with a variety of conditions in a holistic manner both in their own home and in clinic settings and work in close partnership with other primary health care professionals such as GPs, Community Matrons, Community Therapies and other Tier 2 specialised services such as Tissue Viability, Diabetes, and Heart Failure.
