The Homeless Service is an excellent award-winning service (MBE award, two-time Winner of Lammy Award, Regional Winner of Health and Social Care Award for Improving Health and Reducing Inequalities, Winner of GSTT Care Award, Winner of Ab Fab Award) and friendly team delivering services to those experiencing homelessness. The post holder will be predominantly working in non-NHS sites such as hostels, day centres and street nursing when required and will be expected to travel to the Gracefield Gardens office when needed. We offer flexible working opportunities.

We are currently recruiting to a Band 6 Community Nurse Specialist (Adult) secondment or fixed term post, with potential for development. This post is to provide exceptional high-quality nursing care for those who are at risk of homelessness or rough sleeping in Lambeth, Lewisham and Southwark. This is an exciting opportunity to work with a marginalized cohort with complex needs, working in a multidisciplinary team with experts who are leaders in the specialty of Inclusion Health. The new team member will be supported by senior staff, who have many years of experience and knowledge. This exciting role will ensure that vulnerable adults are able to access the healthcare and support they need. Prior to applying for a secondment, check with your line manager.

Main duties of the job

The Health Inclusion Team works across Lambeth, Southwark, and Lewisham to provide a high quality comprehensive specialist nurse service for clients who have difficulty accessing primary care services, refugees and asylum seekers, homeless clients, and clients with addictions and high users of A&E. The Health Inclusion Team aims to assist these clients to access mainstream health services.

Key services provided are:

  • Assistance with GP registration,
  • Health screening (TB, blood borne virus screening, sexual health screening, baseline routine blood tests)
  • Comprehensive health assessment
  • Chronic disease management
  • Wound care
  • Case management of complex clients
  • Referral to other services as appropriate

Detailed job description and main responsibilities

The post holder will run health screening clinics independently and jointly with other team members. All nurses within the team will at times work across all the areas of homelessness, refugees, asylum seekers and addiction clients.

The post holder will also be covering other types of accommodation for asylum seekers such as hotels for asylum seekers and bridging hotels for resettlement families and individuals and refugee services.

Skills

  • Ability to assess, plan, implement and evaluate specialist care delivered to patients/clients and give relevant advice/information to patients and relatives
  • Good communication skills and ability to deal with sensitive information
  • Demonstrates ability to influence and negotiate with others

Experience

  • Previous post registration experience of working with marginalised groups
  • Experienced and competent in the management of patients within the speciality
  • Significant post qualification experience of working in the community, and demonstrable experience of interagency and collaborative working
  • Experience of delivering patient advocacy
  • Experience of outreach and health promotion

Qualifications

  • BSc/1st degree (health related) in Nursing or equivalent qualification/training and experience
  • Evidence of professional/clinical knowledge in area supplemented by specialist clinical, managerial training and CPD Mentorship Course
  • Phlebotomy (or willingness to undertake relevant training)
