Job description

We are seeking an enthusiastic and dedicated Band 6 Nurse to join our Intensive Support Team (IST) for adults with learning disabilities in Peterborough. This team offers an opportunity to showcase your experience and expertise in working with individuals in crisis.

The ideal candidate must have NMC Registration as a Learning Disability Nurse (RNLD) or Mental Health Nurse (RNMH). Practical skills, lived experience supporting individuals with learning disabilities exhibiting behaviours of concern and communication needs, and experience in holistic assessments, report writing, and understanding relevant legislation are essential.

You will join a team of highly motivated professionals valuing person-centered, multidisciplinary working, and continuous skill development to meet service users' needs holistically and creatively.

Our service supports adults with diverse, complex needs, aiming to help them remain in their community during crises through mental health support, therapy, and treatment. We facilitate transitions from out-of-area placements and collaborate closely with services supporting young people to prevent unnecessary relocations.

The community nurse role within the IST involves supporting individuals with complex needs experiencing crises requiring intensive intervention. Responsibilities include triaging referrals, gathering information, liaising with colleagues, obtaining consent, recording data on SystmOne, and sharing information with relevant agencies.

You will also develop and deliver bespoke person-centered training, contribute to formulation development, and participate in writing and training on Positive Behaviour Support Plans (PBS).

Cambridgeshire and Peterborough NHS Foundation Trust is committed to high-quality, compassionate care to improve health and wellbeing, supporting individuals to lead fulfilling lives. Our services span inpatient, primary care, community mental health, forensic mental health, learning disabilities, and more.

We seek high-caliber candidates who share our values and encourage applications from all community sectors, especially underrepresented groups including those with long-term conditions, ethnic minorities, and LGBTQ+ communities.

Please note we may close the advert early if sufficient applications are received. Sponsorship is not available for all roles; applications requiring sponsorship may be rejected.

For more information, visit www.cpft.nhs.uk. The role requires independent travel around the county with a full UK driving license and access to a vehicle, as public transport is unreliable for service needs. Confirm in your application that you meet these criteria.

Refer to the attached job description and person specification for full responsibilities, which include:

  1. Participating in delivering specialist LD nursing services with MDT support.
  2. Leading health needs identification and treatment for adults with learning disabilities.
  3. Sharing information appropriately with families, carers, police, probation, GPs, and partner agencies.
  4. Providing support and guidance to junior staff and support workers.
  5. Developing and delivering training to stakeholders.
  6. Understanding trauma and applying a trauma-informed approach.
  7. Managing personal caseloads autonomously to ensure quality care.
  8. Developing risk assessments, complex condition assessments, and PBS plans, including challenging behaviour, forensic issues, palliative care, mental health, and epilepsy.
