Community Nurse (Forest of Dean)

We are currently seeking nurses possessing the highest standards of care and compassion in order to aid, coach and support our diversepopulation of patient requirements here in the Forest of Dean.

As a Registered Community nurse, you will be placed in the privileged position of caring for patients in their own individualised environment and responsible for the implementation and undertaking of patient centred care.

Our collaboration with our Integrated Community Team colleagues is something we pride ourselves on within the Forest of Dean locality. Inaddition, Gloucestershire Health & Care Foundation Trust Community Nurses are placed in possession of: -

o Individualised development planningo A supportive diverse management structureo Funded university accredited courses (relevant to post in line with individual nurse's career progression wishes and agreed upon withline management)o Autonomy and development to manage own workload and take responsibility to manage a small caseload of patients in partnershipwith line manager

To excel in this role, you will require excellent communication skills, the courage to make clinical decisions and a willingness to develop yourpotential. Please note a full driving and use of a vehicle for work purposes are essential for this role as a Community Nurse.

Main duties of the job

You will require current NMC registration, post-registration experience is helpful but not essential and we have an excellent preceptorshipprogramme for newly qualified nurses. You need to be able to work over a shift system which includes early and late shifts across 7 days including weekends and bank holidays.

Job responsibilities
  • Assess patient and carers needs through strength based assessment,
  • Develop individual care plans in partnership with patients and carers. This will include information, advice, sign-posting and education.
  • Regularly evaluate care plans to ensure they are up to date and appropriate, reporting clinical changes or areas of concern to the appropriate member of the team.
  • Provide skilled nursing care interventions, support and advice to colleagues, patients and carers.
  • Work to reduce unwarranted variation in clinical practice, providing evidence based interventions which effectively enable people.
  • Discuss issues relating to care and support of individual patients with other disciplines and agencies, and refer to those who can better meet their needs.
  • Collaborate with colleagues in the Integrated Community Teams and other aligned services to enable people within their communities, maximising their potential and promoting their independence.
  • Administer and monitor medications consistent with policies and procedures.
  • Contribute to the prioritisation of workload, taking responsibility for a delegated caseload of patients.
  • Assist in providing consistently high standards of evidence based care that meets the patients requirements and satisfaction.
  • Maintain accurate patient documentation on the electronic clinical recording system.
  • Respond appropriately to feedback from patients and carers about the service, using the open approach of the organisation and within Duty of Candour expectations.
  • When suitably experienced, to act as a Named Nurse, providing care oversight for a defined cohort of patients managed in the ICT Community Nursing caseloads.
  • When suitably experienced, to act as a clinical triage nurse within the centralised Community Nursing referral centres, providing clinical review of patients and engaging with health and care professionals to enable effective patient flow and referral management.

Person Specification
QUALIFICATIONS
  • Registered Nurse with NMC
  • Degree or Diploma in Nursing or equivalent
  • Willingness to undertake additional training as required.
  • Full driving licence and use of vehicle
LENGTH AND / OR NATURE OF EXPERIENCE
  • Experience of working without direct supervision
  • Experience of working with student learners in practice
  • Previous experience of community nursing practice
  • Evidence of post-registration education & training.
  • Holistic health assessment course (or equivalent)
  • Teaching & assessing qualification
PROFESSIONAL / MANAGERIAL / SPECIALIST KNOWLEDGE
  • Ability to work on own initiative, assess patients competently and report back to Team Leader (or appropriate health care professional) relevant changes in patient's condition.
  • Ability to effectively manage the workload, prioritise and delegate work in the absence of the Team Leader - District Nurse / Senior Community Nurse
  • Ability to liaise effectively within the Primary Health Care Team and with other professionals / agencies
  • Demonstrable skills in risk assessment and management
