This roleas a band 5 Community Staff Nurse
Or as part ofthe evening team working 18:30-23:30 over 5 evenings a week.
The team is acountywide team which is centrally based in Droitwich with satellite bases inEvesham, Bromsgrove and Kidderminster, (this post is primarily based in Droitwichcovering South Worcestershire) they visit patients in their own home andsupport them in remaining at home and preventing admission to hospital andsupporting early discharge from hospital.
The postis all worked as unsocial hours hence picking up pay enhancements for all hoursworked which makes this post more attractive. more money for less hours!!

Asa community Nurse working with the Evening Team most of the work allocated toyou will of an unplanned nature. The service works as an on-call servicecovering the County of Worcestershire taking direct referrals from patients,111, Ambulance Service and discharging hospitals. You can expect to have acomprehensive induction and work alongside peers until you are confident tothen work alone.
The evening service nurses primarily work alone, and havework allocated via a handheld tablet by the triage nurse based at Droitwich.You will be able to start your visits from home if the geographics dictates.
Thepatients that are seen during the evening and night vary immensely and no 2shifts are the same. For example, we see catheters, palliative care, woundcare, administration of medication, diabetics, and patients requiring intensivesupport rehabilitation following discharge from hospital or admissionprevention. As a team we often undertake initial assessments of patient in thecommunity.
As part of a new initiative,we are working closely with West Midlands ambulance service and 111 and assistin responding to urgent Care response. These can be patients that have fallen,or patients requiring further assessment to aid prevention in admission tohospital.

You will bejoining the Evening element of one of our dynamic and supportive NeighbourhoodTeams to provide a 24-hour admission avoidance service to the population ofWorcestershire which encompasses both urban and rural areas.
The successfulcandidate will deliver independently a high standard of nursing care forpatients in their own homes. Ensuring that care provided maintains theindividuals privacy and dignity, values diversity and respects cultural andindividual rights and choices.
We are lookingfor candidates with good communication skills, a flexible approach andenthusiasm to work within the team. Youshould be a committed, highly motivated and forward-thinking individual who isa good team player.
The successfulcandidate will also assist the team to develop and achieve team andorganizational objectives. They will also participate in staff meetings andtraining & development activities as necessary to ensure their skills andknowledge are up to date.
Due to thenature of the post the applicant must be able to travel independently and haveaccess to a suitable vehicle for business purposes, and hold a valid UK drivinglicence. Whilst a base will be identified, staff can be asked to work anywherewithin the Business Unit or Service if necessary.
