A community healthcare provider in Warrington is seeking a Community Nurse Discharge Facilitator to support safe and timely patient discharges from hospital. The ideal candidate will work collaboratively with multidisciplinary teams, exhibiting strong clinical judgement, communication skills, and initiative. We are lookingto recruit a discharge facilitator on a fixed term basis of 9 months to covermaternity leave this post will be working within the Transfer of Care Hub atWarrington Hospital.
The Band 6Discharge Facilitator plays a pivotal role in ensuring the safe, effective, andtimely discharge of patients from hospital to community or other care settings.Working closely with multidisciplinary teams (MDT), patients, families, andexternal partners, the postholder helps coordinate discharge planning fromadmission through to discharge, with a focus on reducing delays and enhancingpatient experience.
This rolerequires strong clinical judgement, excellent communication and negotiationskills, and a proactive approach to problem-solving. The Discharge Facilitatorwill act as a key link between ward teams, social care, community services, andcontinuing healthcare teams, ensuring that all necessary arrangements are inplace to support patients ongoing care needs.
To work as part of the multi-disciplinary IntegratedDischarge Team in relation to identifying patients who are palliative fasttrack patients and are ready for discharge from hospital.
To ensure a quality seamless service for patientsand carers who require nursing services on discharge from hospital
To use own initiative and to be accountable fororganising and prioritising own workload to ensure deadlines are achieved.
Toundertake assessments in relation to discharge planning.
Flexible working will be considered for all roles.

1. To maintain own Personal and professional development and clinical competencies via on-going Continuing Professional Development (CPD)
2. To maintain a personal professional portfolio by collecting evidence of personal development through reflective practice.
3. To ensure that all Statutory and mandatory training is attended in accordance with Trust policies.
4. To work within Nursing and Midwifery Council (NMC) guidelines, and support the Locality Manager in ensuring these are adhered to.
5. To encourage others to also abide by the NMC Code of Professional Conduct.
6. To bring any professional practice issues to the attention of the District nurse coordinator.
