We provide a range of Community Services, and the Hertfordshire Division is recruiting!

Position: Community Nurse

An exciting opportunity has arisen for a Community Nurse to join our Community Adult Planned Care team based in Watford. The team includes Community Nurses, District Nurses, Community Paramedics, Physiotherapists, Occupational Therapists, and Health Care Assistants. Our Community Nursing team operates from 8am-6pm, 7 days a week, aiming to provide safe, patient-centred, evidence-based care.

The successful candidate will undergo a 12-week induction programme, which for newly qualified RNs, complements your CLCH preceptorship. This induction includes working with the wider MDT to enhance your skills and knowledge, with regular support from Professional Development Nurses and your Team Leader. We offer various development opportunities, and upon completing the Community Nursing Pathway, support is available to apply for the Deputy Team Leader pathway.

We offer flexible shifts, and the successful candidate will be required to work a minimum of 3 weekend days per 4-week period.

Responsibilities:
  • Provide skilled nursing care under supervision, in community settings.
  • Deliver holistic assessments and person-centred nursing services.
  • Care for housebound patients, coordinating their needs to prevent unnecessary hospital admissions.
  • Facilitate self-care for patients with long-term conditions, health promotion, and innovative service activities.
Skills and Qualifications:
  • Competence in chronic disease management, end-of-life care, continence promotion, and wound management.
  • Specific skills include phlebotomy, medication administration via syringe drivers and IV therapy, PEG management, urinary catheterisation, and tracheostomy suctioning.
