Job summary

A leading healthcare trust is seeking a qualified nurse to provide care for patients with Long Term Conditions. The role requires a First Level NMC Registration and extensive experience in community nursing. Successful applicants will demonstrate excellent communication and teaching skills, as well as effective time management to ensure high-quality patient care.

Qualifications

Essential criteria

  • First Level NMC Registration
  • Recognised teaching assessment qualification/accreditation
  • Diploma in Health related subject
  • Evidence of recent CPD
  • Evidence of working with patients with Long Term Conditions

Desirable criteria

  • Documented evidence of having undertaken relevant study in relation to Long Term Conditions
  • First Degree or working towards
  • Leadership qualification
  • Non-medical prescribing qualification (V300) or willing to undertake

Skills/Knowledge

Essential criteria

  • Excellent communication skills (written & verbal)
  • Effective time management
  • Knowledge of current Health and Social Care Legislation affecting people with Long Term Conditions
  • Teaching Skills for patients, carers and staff

Desirable criteria

  • European Computer Driving License (ECDL)
  • Evidence of research or audit projects, presentation skills within roles
  • Evidence of participation in Multidisciplinary assessments
  • Experience in working with a cohort of patients with Long Term Conditions, for example: Diabetes, Dementia, Cardiac, Respiratory, Renal

Experience

Essential criteria

  • Significant post registration experience
  • Demonstrate a working knowledge of community nursing, case management and management of a patient within a community setting

Desirable criteria

  • Evidence of working with Multidisciplinary partnerships and with other agencies

You must have appropriate UK professional registration.
