Overview

The community nursing team across Peterborough are currently offering opportunities for full time and part time hours working within CPFT. We provide a service 7 days a week, working closely with other community services and PCN’s to provide high standard holistic care to the population of Peterborough. You will have opportunity to develop yourself, with extensive learning and development opportunities.

Travel & Driving
  • Required to travel independently around the county meeting strict time deadlines. You must hold a full UK driving licence and have use of a vehicle. Public transport is not a reliable form of transport for this role.
Responsibilities
  • Work as an autonomous practitioner within the integrated community nursing team.
  • Accountable for assessing, interpreting, planning, implementing and evaluating treatment to patients within professional guidelines, with the support of the multidisciplinary team.
  • Keep the patient at the centre of care, ensuring patient and carer participation in decision making.
  • Provide support and education to peers, new staff, non-registered staff and students.
  • Provide a high standard of care and nursing intervention within a patient's own home, including lone working with access to Specialist and Advanced Practitioners when required.
  • Work as part of a 24/7 service.
  • Identify patient needs, agree goals and provide appropriate holistic nursing interventions, refer to other services, and provide or order equipment.
