Job summary

HCRG Care Group is seeking a Community Nurse (Band 5) to provide high-quality, person-centred nursing care in patients' homes from Beacon House, Devizes. 37.5 hours per week.

You will work within national and local policies, collaborating with GPs and multidisciplinary teams to assess needs and promote independence. You will be supported by a Learning Enterprise for CPD and development, with access to NHS Pension and My Reward Hub benefits.

Qualifications

  • NMC registration (1st level) is required.
  • Ability to work autonomously within a multidisciplinary team.
  • Strong communication and organisational skills.

Responsibilities

  • Assess, plan, deliver, and evaluate nursing care in partnership with patients, carers, and the multidisciplinary team.
  • Monitor patient progress and escalate changes in condition as needed.
  • Prioritise and manage a caseload in line with patient need.
  • Maintain accurate records using electronic and paper-based systems.
  • Deliver evidence-based care and contribute to quality improvement initiatives.
  • Promote health, wellbeing and independence for patients and carers.

Job Introduction

As a Community Nurse Band 5, you will deliver high-quality, person centred nursing care to patients in their own homes, working in line with national and local policies, professional standards, and the NMC Code. You will work closely with patients, families, GPs, and multidisciplinary colleagues to assess needs, plan and deliver care, promote independence, and support positive health outcomes. Main Responsibilities
  • Assess, plan, deliver, and evaluate nursing care in partnership with patients, carers, and the multidisciplinary team.
  • Monitor patient progress, recognising and escalating changes in condition appropriately.
  • Prioritise and manage a caseload in line with patient need.
  • Maintain accurate and timely patient records using electronic and paper-based systems.
  • Deliver evidence-based care and contribute to quality improvement initiatives and clinical audits.
  • Promote health, wellbeing, and independence for patients and carers.
  • Participate in team meetings, handovers, clinical supervision, appraisals, and mandatory training.
  • Support the learning and development of students and colleagues.
  • Contribute to maintaining a safe environment through risk assessment, incident reporting, and adherence to infection prevention and control standards.
  • Ensure the safe use, maintenance, and management of clinical equipment and stock.

The Ideal Candidate

You will be a NMC/1st level registered Nurse, you will have the ability to work both independently and also as part of a wider multidisciplinary team. It is expected that you should have strong communication and organisation skills and a flexible approach which will ensure you deliver positive experiences to patients at every stage of their healthcare journey. You will have a full UK driving license with access to a car for work purposes with Business insurance for the vehicle used. You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
