Job summary

NELFT NHS Foundation Trust is looking for an enthusiastic Nurse Associate or Assistant Practitioner to join our Community Health Care Service in Barking & Dagenham. You will deliver holistic care to patients in their homes and residential care settings, under supervision, within your agreed competency.

You'll work as part of an integrated team, liaising with GPs and other professionals. A valid NMC registration and a full UK driving licence are essential to travel between sites and provide

Qualifications

• Registered nurse associate or assistant practitioner with NMC registration.
• Ability to work effectively in a multi-disciplinary Primary Health Care Team.
• Excellent verbal and written communication skills.
• Car driver with full UK driving licence and access to a car.

Responsibilities

• Deliver treatment, advice and teaching within your level of competency.
• Provide holistic care to patients in their homes and residential settings.
• Liaise with GPs, other professionals and agencies; travel between sites as required.
• Work weekends and some bank holidays.
