The post is in Cornwall Partnership NHS Foundation Trust’s Arbennek Community Nursing Team. It involves independently assessing, planning, delivering and reviewing nursing care in patients’ homes, while supporting patients and carers to make informed choices and maintain independence. The community-based work includes frequent travel across Cornwall, including places not served by public transport, and collaboration with locality teams and wider health and social care partners. An adult nursing qualification and NMC registration are required; students completing their qualification this calendar year may be considered for a conditional offer.

What the job involves

• Assess, plan, implement and evaluate care for patients, carers and relatives in their homes without direct supervision
• Work with patients and carers to provide support and advice, promote informed choice, and help maintain independence and quality of life
• Provide autonomous, evidence-based care in line with competencies, training records and organisational policies, procedures and guidelines
• Support the Community Nurse Team Leader and locality teams flexibly according to service needs
• Travel frequently to locations throughout Cornwall, including areas inaccessible by public transport
