We are seeking a dedicated and compassionate Community Nurse to join our healthcare team. Duties

  • Deliver nursing care to patients in their homes or community settings, ensuring adherence to established care plans.
  • Assess patients’ health status and monitor their progress, making necessary adjustments to care plans as required.
  • Collaborate with multidisciplinary teams to coordinate patient care and ensure comprehensive service delivery.
  • Educate patients and their families about health management, medication administration, and lifestyle modifications.
  • Maintain accurate and up-to-date patient records, documenting all care provided and any changes in patient condition.
  • Demonstrate strong time management skills by prioritising tasks effectively in a fast-paced environment.
  • Provide leadership and support to junior nursing staff and volunteers within the community setting.

Qualifications

  • Registered Nurse (RN) qualification with a valid nursing licence.
  • Proven experience in community nursing or a similar healthcare setting is preferred.
