Wight Healthcare is seeking a Registered Nurse to join the Integrated Community Nursing Service on the Isle of Wight. You will deliver high quality nursing care to patients in their own homes, contributing to safe, compassionate and evidence-based care while upholding our values.
You will work across locality teams, collaborating with GPs, Practice Nurses, and other health professionals, and you must be able to travel across the island using your own vehicle with business insurance coverage.
The postholder will work as part of the Integrated Community Nursing Service on the Isle of Wight, delivering high quality nursing care to patients in their own homes as part of a rapidly developing community based model. As a Registered Nurse, they will contribute to safe, compassionate and evidence based care, supporting the Trust's commitment to excellence and continuous improvement.
The role involves working flexibly across three locality teams and collaborating closely with a wide range of partners, including Earl Mountbatten Hospice, Urgent Community Response, Allied Health Professionals, GPs, Practice Nurses, Specialist Nurses, mainland hospitals and voluntary or statutory agencies. The postholder will use strong interpersonal and clinical skills to assess, plan and deliver care, ensuring patients receive the right support in the right place.
For this role you are required to hold a full UK driving licence and have access to vehicle that you can use for work that has business insurance cover, as it involves travel across the island. The postholder must be able to work independently within a busy community environment, demonstrate evidence-based practice, and uphold the Trust's values in all interactions with patients, families and colleagues.
