A community health organization in Plymouth seeks a professional to join their Community Mental Health Team. This role focuses on improving the physical health of individuals with Severe and Enduring Mental Health Illness (SMI) through various interventions. As afully integrated member of the Community Mental Health Team, the post holderwill support the implementation of the Community Mental Health Framework with afocus on supporting the delivery of interventions aimed to address the physicalhealth inequalities faced by people with Severe and Enduring Mental HealthIllness (SMI) The post holder will seek to influence, improve, and promotepositive lifestyle choices and behaviours, which will lead to better healthoutcomes and a reduction in preventable ill health. Key lifestyle behavioursinclude being smoke free, eat well, move more and drink less.

All Livewell staff are expected to be able and willing to work across a 7-day service.
The Community Mental Health Team work with individuals who experience complex mental health difficulties as well as other challenges, which at times prevent them from accessing mainstream services. This role will involve supporting the physical health needs of our patient group both in a clinical setting as well as outreach work within the community and within peoples own homes.
The post holder will work alongside mental health nurses, health improvement practitioners, occupational therapists, support workers, psychologists, substance misuse worker, assistant practitioners, social worker and social inclusion worker.

MAIN DUTIES/RESPONSIBILITIES
Carryout specialist one-to-one and group interventions for behaviour change.
Undertakephysical health monitoring, including outreach for those who are difficult toengage.
Towork in partnership with members of the multi-disciplinary team to promote thephysical health and wellbeing of service users within specialist mental healthservices.
Liaisewith partners to help reach target populations and communities.
Activelyseek out Hard to Reach populations and help them to engage in healthlifestyle initiatives.
Supportthe development and delivery of training programmes for other professionals /interestedparties internally and externally.
Supportthe development, promotion and co-ordination of health activities and othercommunity engagement events.
Signposting/referralinto the One You Plymouth service and/or partner services such as IPS, andHarbour.
Contribute at appropriate network meetings and multi-disciplinary teammeetings.
Monitorand evaluate outcomes via processes, procedures and systems.
Givefeedback on performance and local intelligence to senior management team.
Ensurethat all stakeholders receive up-to-date information on a wide range of healthimprovement topics.
Contribute to thecommunications and marketing strategy by collecting case studies and photos.
Develop fact sheets andresources.
Contribute to researchand development as directed.
Act as a HealthChampion, Making Every Contact Count.
Be up to date with arange of health improvement topics through competency-based learning.
Support the nursingteam in daily medication management
Support the team indelivering medication interventions such as depot medication administration
Assess and utilizerelevant clinical tools to support with monitoring side effects of medicationand addresses as clinical indicated with the wider team
Complete relevantmental health clinical competence to support practice.
4.1 Responsibility forPeople Management

4.2 Responsibility forfinancial and/or physical resources
Collect information regardingpurchasing and invoicing to support managers budgetary requirements.
4.3 Responsibility for administration
Ensurerecords are up-to-date and stored as per record keeping policy.
Responsiblefor developing training and psycho-social education packs, presentation,leaflets and handouts.
4.4 Responsibility forpeople who use our services
Responsiblefor giving advice and support to service users and/or family members/carersover the phone, face to face and digitally.
Responsiblefor case load management.
Requiredto attend external events, speaking to members of the public or eventadministration.
Maybe required to present information at steering groups, forums, events, seminarsand conferences.
4.5 Responsibility for implementation of policy and/or service developments
The post holder will contribute to service development and implement changes to established working processes as appropriate.

Designand produce health promotion displays, presentations and publicity.
Establishand maintain effective channels of communication across Livewell Southwest andthe wider community.
Marketand promote the One You branding and underpinning health improvement messageswith a particular focus on priority areas.
Seekopportunities to raise awareness of health promoting messages through the Commsteam and wider partners such as national campaigns etc.
6. PHYSICAL DEMANDS OF THE JOB
Thisrole requires the post holder to engage in physical activity.
Thereis likely to be frequent sitting and standing.
Frequenteffort to lift and manual handing of equipment for health events, clinics andtraining etc.
Keyboardwork is likely throughout the working day.
Concentrationrequired when delivering client support and health improvement interventions.
Concentrationrequired for reading documents and assimilating knowledge.
Thepost holder will be required to keep up to date with public health documentsand NICE guidance.
Sometraining delivery may be emotive E.g.Suicide Prevention and Mental Health First Aid. The post holder may needto seek support from colleagues or line manager on such occasions.
Someone to one and group interventions may be upsetting for the client. The postholder may need to seek support from colleagues or line-manager on suchoccasions.
Thepost holder will use VDU for high proportion of time. However all staff will beentitled to a workstation assessment and VDU check.
Willwork in a variety of settings including home, office, community centres,outdoors at health events, traveller sites and inpatient settings.
Servicecore hours are 0900 to 1700.
Engagingservice users in treatment interventions in at times unsanitary environments
