Job Summary

To work as an integral member of the Haringey Learning Disabilities Partnership (HLDP), working primarily within the mental health pathway (although may have some cases on physical health pathway), monitoring and supporting individuals with a learning disability, autism, and an additional mental health need and/or behaviours which challenge. The role involves supporting Service users and carers/providers to proactively address the mental and physical health needs; this involves undertaking assessments, undertaking health screening, formulating robust risk management plans, providing education & training, supporting access to mainstream services, promoting reasonable adjustments, and acting as a care-coordinator when required.

Main duties of the job

  1. To undertake eligibility assessments for access to a specialist Learning disability service.
  2. Undertake a variety of assessments which inform the development of a person-centered care plan.
  3. To identify specialist behaviour support needs and support the design and implementation of appropriate interventions in consultation with the wider MDT.
  4. Undertake CPA care co-ordination.
  5. To undertake and support mental capacity assessments and best interest decision making.
  6. Provide advice and support to families, carers, health and social care professionals and service providers regarding meeting the support needs of people with learning disabilities and mental health needs and/or challenging behaviour.
  7. Work seamlessly with the AIT (Assessment & Intervention Team) when there is a risk of placement breakdown or hospital admission.
  8. Attend/chair a variety of meetings and participate in care and treatment reviews as appropriate.
  9. Undertake CHC assessments and be able to present each case to panel, when required.
  10. Participate in teaching and learning events, and the education and induction of staff within the team, including positively supporting students on placement within the team.

Qualifications
Essential
  • Registered Nurse (RNLD or RMN)
  • Computer skills
  • Experience of supervising and assessing student nurses
  • Teaching / Presentation experience
Desirable
  • Specialist knowledge regarding the mental and physical health support needs of people with learning disabilities
  • Recruitment & Retention Training
  • Educated to Diploma level or higher
  • Additional qualifications (Diabetes, dementia, pressure area care, working with offenders, Autism)
  • ECDL - or Computer based courses
  • Clinical supervision training
Experience & Knowledge
Essential
  • Knowledge of CPA
  • Knowledge of MCA
  • Knowledge of MHA
  • Care co-ordination experience
  • Knowledge of common health conditions e.g. Epilepsy, Diabetes, autism, mental illness
  • Experience of working with people with a learning disability who have super imposed mental health issues/autism/challenging behaviour
  • Experience of complex physical health casework
  • Experience of supervising/coaching/mentoring students in practice
  • Research awareness
  • Knowledge of best practice in safeguarding vulnerable adults and child protection
  • Knowledge of control of infection strategies
  • Working knowledge of the application of professional Code of Conduct
Desirable
  • Experience in education of nurses/health practitioners and/or multi-professional groups
  • Good knowledge of the aetiology and impact of learning disabilities
  • Working with families and carers
  • Experience of working within Acute In Patient settings
