A health organization is looking for a Band 5 Community Nurse to join its Community Nursing Team in Lampeter. In this permanent full-time position, you will manage a caseload of patients, providing high-quality care in their homes. The role requires the ability to travel between sites and effective communication skills. Welsh language skills are desirable, but applications are welcome from all candidates. This position offers a salary range from £31,516 to £38,364 per annum.
We have an exciting opportunity for a Band 5 Community Nurse to join our Community Nursing Team in Lampeter/Llanybydder.
The role will allow you to support the management of a delegated caseload within the Community Nursing Team providing high quality and complex care management to patients in their own homes, with the guidance and support of an experienced District Nurse Team Leader.
The post holder will ensure high standards of care are delivered to patients and their families/carers within the home or clinic environment. This includes liaising with staff, patient/carers and other agencies/organisations as required to ensure appropriate delegation of duties and risk management, whilst ensuring effective communication between those involved in the care package.
Ceredigion's Community Nursing Teams have a wide-ranging nursing skill mix and pride themselves on supporting professional development and improving high standards of community practice in line with current health policy.
This is a permanent position, worked over a 7-day period including weekends and bank holidays covering an 8am-8pm service.
The post holder will need the ability to travel between sites in a timely manner.
The ability to speak Welsh is desirable for this post: English and/or Welsh speakers are equally welcome to apply.
