Overview

Are you a nurse looking for a supportive team and the opportunity to develop new skills? This is an exciting time to join the community nursing service in West Herts. We are developing new community pathways which will support the delivery of more care in people\'s homes, and to enable us to do this we have a range of new learning and development opportunities available through the CLCH Academy.

Responsibilities
  • Provide skilled nursing care working under the supervision of the Community Nurse Team Lead and Deputy Team Lead to patients in a variety of community settings.
  • Undertake community nursing duties involving delivery of nursing services and holistic assessments of patients health needs using a person-centred approach, providing quality and seamless nursing care to housebound patients, coordinating their care needs as appropriate, and helping to avoid unnecessary admission to hospital or long-term care institutions.
  • Facilitate self-care for patients with long-term conditions, promote health, and participate in innovative activities and projects that enhance patient service delivery.
  • Deliver skilled nursing care based on best practice and clinical evidence, including chronic disease management, end-of-life care, continence promotion, and wound management. Specific skills include phlebotomy, administration of medications via syringe drivers and intravenous therapy, PEG management, urinary catheterisation, and tracheostomy suctioning.
  • Act as a Named Nurse with the support and supervision of the DN Team Lead, maintaining holistic health needs assessments, care planning, evaluation, and appropriate implementation of health care to ensure continuity of care following referrals from acute care, rehabilitation units, GPs and other agencies.
  • Undertake comprehensive assessment of physical, psychological and social care needs using the Single Assessment Process, including needs of carers, and provide individualised, person-centred care plans that monitor and promote self-care to prevent unnecessary hospital admissions.
  • Act as the patient\'s advocate, facilitate the patient\'s own choices regarding nursing care, and promote independence and self-care where appropriate.
  • Identify and record new problems and other relevant information, reporting back to the DN Team Lead as appropriate.
  • Maintain accurate, comprehensive, and contemporaneous records in accordance with Trust and NMC guidelines.
  • Work with people with complex health needs and support students and other learners (e.g., HCAs teaching others in the practice environment).
  • Engage in general assessments using evidence-based care and apply the Nursing Process, care planning, and sound record keeping.
  • Provide holistic care that involves the multidisciplinary team.

  • Skills and Knowledge
  • Essential criteria: Knowledge of out-of-hospital care and the role of the Community Nurse; ability to listen effectively and provide constructive feedback; knowledge of how Equal Opportunities can be implemented in practice; awareness of data management in the NHS; knowledge of risks in community work; knowledge of long-term condition management (e.g., diabetes) and self-management concepts; knowledge of infection control; knowledge of general wound care; literacy and numeracy skills.
  • Desirable criteria: Knowledge of recent government legislation and changes in community care delivery; experience training other students; participation in audit, research projects, or service development; post-registration experience in an NHS community environment.
