An exciting opportunity hasarisen for a Community Nurse to join the Neighbourhood Team in Bromsgrove for afixed term 12 month period. This will give you the rewarding opportunity toprovide optimal care to patients in their own home. Shift patterns are between07.30-18.30. We are looking for a highly motivated, flexible team player with astrong, quality driven patient focus and an ability to work through change toembed new ways of delivering care. The successful candidate will have experience in working within amulti-disciplinary team. Inreturn we offer a great team environment, learning and developmentopportunities and supervision support.
The teams consist of CommunityNurses, Physiotherapists, Occupational Therapists, Healthcare Assistants andadmin staff with GP, paramedic and social care colleagues working closelyalongside. This enables the teams deliver planned nursing, therapy and admissionavoidance/supported hospital discharge interventions to the adult patientpopulation of Redditch and the surrounding areas.

A typical day ascommunity nurse may start with greeting a diabetic patient and administeringtheir insulin, followed by supporting patients at the end of their life toensure gold standard care is delivered. You may then visit patients to dress aleg ulcer or post op wound, followed by changing a catheter, to flushing apatients central line. Following a morning of visits you will return to theoffice for your team handover, this gives you the opportunity to discusscomplex situations with your colleagues and undertake clinical supervision. Youmay then be called out to an urgent care response visit where a patient needsto be seen and assessed within 2 hours. No day is the same within communitynursing and each day is as rewarding as the next, whether youve healed a legulcer or given a patient the best possible care at the end of their life. This willbe achieved through a combination of hands on clinical skills and knowledge totreat, educate and support patients on a designated caseload and by workingwith patients and their carers to assess, plan and implement appropriatepackages of care.
Regularly takeresponsibility for the caseload in the absence of the Senior Community Nurse.
