Join an 'Outstanding' rated Trust as a Band 5 Community Nurse in the new Thanet Health Hub. You will be providing complex nursing care, particularly for elderly patients or those with additional healthcare needs, in a proactive community-focused environment. The role supports the NHS's vision of community-based care, emphasizing patient-centered service and professional growth within a diverse organization.
We are looking for passionate and skilled Band 5 Community Nurses to join our Thanet Community Nursing Teams, to be based at our brand new £10million state-of-the-art Thanet Health Hub.
Our new hub directly supports the Governments new 10-year plan, that places community care at the heart of our health system.
This plan represents one of the most significant shifts in NHS history: moving care from hospitals into neighbourhoods, from reactive treatment to proactive prevention. And here at Kent Community Health, we're right at the forefront.
Planning to open in Autumn 2025, our new Thanet Health Hub will consist of an NHS community diagnostic centre, a range of community NHS services, including community nursing, relocation of an existing GP surgery and signposting and support to access health and care services provided by voluntary sector organisations, such as Age UK.

Community nurses provide complex nursing care to people at home and in residential care homes.
As a community nurse, you'll assess the healthcare needs of patients and families monitor the quality of care they're receiving be professionally accountable for its delivery.
Your patients could be any age, but they'll often be elderly, while others may have been recently discharged from hospital, be terminally ill or have physical disabilities
