Job summary

Derbyshire Community Health Services NHS Foundation Trust in Derbyshire is recruiting a registered nurse to join Amber Valley Community nursing service, supporting housebound patients. Our teams are friendly and supportive, meeting the needs of residents in Amber Valley.

You will manage referrals, assess, plan, implement and evaluate individualised care, promote self-care with dignity and privacy, and support junior staff and the District Nurse/Senior Community Nurse in the clinical environment.

Qualifications

  • RGN with current registration.
  • Experience of working in the community.
  • Desirable: Long Term Conditions, wound management or End of Life Care experience.

Responsibilities

  • Manage referrals into our services and provide evidence-based care to support patients in their own homes.
  • Assess, plan, implement and evaluate individualised patient care promoting self care wherever possible while maintaining privacy and dignity.
  • Provide advice, guidance and support to junior staff and actively supporting the District Nurse/Senior Community Nurse in the management of the health care environment.
  • Work alongside MDT members including Therapy Teams, General Practice and Specialist Services.

We are looking to recruit a registered nurse to work within the Amber Valley Community nursing service. . We are friendly, supportive teams who work closely together to meet the needs of the housebound population within Amber Valley.

Main duties of the job

The successful applicant/s will be responsible for managing referrals into our services and providing evidence-based care to support patients in their own homes. To assess, plan, implement and evaluate individualised patient care promoting self care wherever possible in an efficient and clinically effective way, whilst maintaining privacy and dignity is vital.

Providing advice, guidance and support to junior staff in the delivery of high quality care and actively supporting the District Nurse/Senior Community Nurse in the management of the health care environment are integral to this role..
