Job summary

Central London Community Healthcare NHS Trust is seeking an enthusiastic physiotherapist to join the team in the community setting. The role includes managing your own caseload and screening/co-ordinating new Stroke ESD referrals, within an interdisciplinary team including OTs, SLTs, psychologists and rehabilitation support workers.

You will assess and treat patients with acquired and progressive neurological conditions, collaborate with GPs, acute hospitals and social care, and participate in

Qualifications

• Assess patients with neurological presentations on Stroke and neuro rehabilitation pathways to develop and deliver individualized treatment programs.
• Work as part of a multidisciplinary team to facilitate positive rehab outcomes and timely discharge planning.
• Undertake evidence-based audit and research to inform physiotherapy practice.

Responsibilities

• Perform physiotherapy assessment of patients with neurological presentations on Stroke and neuro rehabilitation pathways.
• Collaborate with multidisciplinary team to achieve rehab outcomes and coordinated discharge planning.
• Support initiatives to reduce length of stay and provide flexible support on other wards when needed.
• Hold responsibility for own caseload and provide supervision for rotational Staff.
• Contribute to audit and research projects to inform practice within the unit.
• Participate in a 7-day service covering weekends and bank holidays on a rota.
