Essex Partnership University NHS Foundation Trust is recruiting an experienced Band 7 Community Neurology Specialist Nurse with a focus on Parkinson's Disease. The post involves developing and leading a seamless community PD service, with collaboration across Primary Care Networks and local partnerships to improve patient outcomes.

We are looking to recruit an experienced senior nurse to work as a Band 7 Community Neurology Specialist Nurse with interest in Parkinson's Disease.
We welcome applications from experienced or existing Band 7 Parkinson's Disease Specialist Nurses as well as nurses working at Band 6 level aspiring to progress into a specialist nursing role within neurology.
The post holder will develop, implement and evaluate a seamless community PD specialist service ensuring that patients receive the highest standard of clinical care and that specialist advice, education and support to health professionals and patients is provided.
They will work collaboratively with the established Primary Care Networks, ensuring cohesive leadership across the three localities.
The post holder will be an integral part of the West Essex Care Closer to Home model, working in partnership with Social Services and other statutory and non-statutory services to meet the needs of the local population using public health information/population management to support service development.
