You will support women and birthing people with complex needs, promote engagement with maternity services and advocate for those in your care.

Responsabilités

• Manage a defined caseload of women and birthing people with complex social, safeguarding and health needs.
• Promote engagement with maternity services and build trusting relationships.
• Assess, plan, implement and evaluate individualised care from pregnancy through postnatal period.
• Coordinate with Named Midwife for Safeguarding, Perinatal Mental Health Team, Health Visitors, Social Workers and Obstetricians.
• Identify and escalate safeguarding concerns following guidance.
• Champion equity, inclusion and personalised care and reduce maternity disadvantage.
• Work autonomously within local policies and national guidance."
