Job summary

Frimley Health NHS Foundation Trust is seeking a Community Midwife to join our Wexham Park Hospital team. You will deliver compassionate antenatal and postnatal care to women with complex needs within the community, coordinating with safeguarding and social care partners to reduce health inequalities and improve outcomes.

Qualifications

Job Overview

An exciting opportunity has arisen to join our Crystal Team as a Community Midwife at Wexham Park Hospital.

Working alongside a multidisciplinary team, you will provide compassionate, personalised care to women and birthing people with complex social, safeguarding and health needs. You will play a key role in reducing health inequalities, promoting continuity of care, and improving outcomes for some of the most vulnerable families across our maternity service. Based within the community, you will support women throughout pregnancy, birth and the postnatal period while working closely with partner agencies to ensure safe and coordinated care.

Main duties of the job

• Provide high-quality antenatal and postnatal care to women with additional vulnerabilities and complex needs.
• Act as a lead professional, coordinating care and working closely with safeguarding, health, social care and voluntary sector partners.
• Assess, plan, implement and evaluate individualised care plans to support improved outcomes for women and babies.
• Promote safeguarding, public health initiatives and healthy lifestyles.
• Maintain accurate clinical records and contribute to quality improvement and audit activities.
• Support women and families through effective communication, advocacy and compassionate care.

• Current on the NMC register
• Diploma/BSc in Midwifery Studies
• Has completed preceptorship

Desirable criteria

• Registered Nurse
• Qualified mentor / PSPA / SLIP

Experience

Essential criteria

• Be able to demonstrate a knowledge of the 6 C’s
• Able to demonstrate awareness of research related to midwifery practice
• Experience of looking after vulnerable patients
• Epidural management Perineal experience.

Desirable criteria

• Has participated in undertaking midwifery research and audit
