Job summary

Tees Esk and Wear Valleys NHS Foundation Trust is seeking a Community Support Worker to join the Durham & Darlington Community Rehab and Recovery Team. The role focuses on improving lives of individuals with mental ill health through compassionate, patient-centred community care.

Qualifications

• Diploma in Health and Social Care level 2 or equivalent.
• Care Certificate.
• Literacy and Numeracy level 2.
• Successful completion of Trust's full HCA Framework within 6-month probationary period.

Responsibilities

• Provide patient care under supervision in community settings.
• Assist in safeguarding and health promotion activities for patients and carers.
• Travel across locality and attend patients in homes and community venues as required.
