The NHS Wales Betsi Cadwaladr University Health Board is seeking an experienced Support Time Recovery worker to join the community mental health primary care team in Conwy. You will support service users to regain independence and access local housing, benefits, and education services, while coordinating with GP practices and third-sector organisations.

Qualifikationen

• Certificate in Community Mental Health Care / NVQ Level 3 in care or equivalent (essential).
• Good literacy and numerical skills (essential).
• Desirable qualifications in Health, Social and Care.

Aufgaben

• Provide support to an allocated group of service users and promote recovery in the community.
• Assist service users to meet identified needs and access community resources and services.
• Work independently and liaise with third sector agencies and carers.
• Establish and maintain links with GP practices and multidisciplinary teams.
