Torbay and South Devon NHS Foundation Trust is seeking a dynamic Social Worker to join our integrated Mental Health Team. You will provide appropriate and effective interventions to people with severe and enduring mental health needs, and their networks, helping them meet daily health, social care and wellbeing goals in line with personal recovery.

The role requires experience with mental health clients, community-based working, assessment and care planning, and the ability to manage a complex

Qualifikationen

• Experience of working with people who have mental health problems.
• Experience of working in a community setting in relation to supporting people to achieve and sustain independent living.
• Assessment and care management experience.
