Job summary

Hoop Social Work is seeking a Community Mental Health Social Worker for the Taf Ely area, based at Dewi Sant Health Park, Pontypridd. The role covers a 9-month initial contract with 37 hours per week, and potential for a 4-day week. Office-based with some travel across the Taf area, driving licence and car required.

You will assess, plan and review care, co‑produce treatment plans, complete MCA assessments, maintain records, and work with partner agencies to promote independence and wellbeing.

Qualifications

• A minimum of 6 months' experience working in the UK as a registered Social Worker.
• Current registration with Social Care Wales or Social Work England.
• Three years of referencing history.
• A driving licence and access to a car.

Responsibilities

• Undertaking assessments, care planning and reviews to safeguard adults.
• Co-producing and implementing care plans, acting as a care coordinator.
• Completing mental capacity assessments under the Mental Capacity Act 2005.
• Carrying out WARRN risk assessments while balancing autonomy and safety.
• Working with the Social Services and Well-being (Wales) Act and related measures.
• Maintaining accurate case records, plans and reviews.
