Job description

Lewes neighbourhood mental health team, formerly the ATS, has an exciting opportunity for a social worker to join our developing service. As we transition into community transformation and collaborate more closely with partner organisations, we seek a dynamic, enthusiastic, flexible, innovative, and experienced Social Worker.

We are a friendly, professional service based in the lovely seaside town of Newhaven. Newhaven, a gateway to Europe, is close to the South Downs National Park and Brighton City Centre. Brighton offers a vibrant shopping centre with many independent shops, restaurants, and bars, as well as a seafront and good public transport links to London. The area boasts many beautiful beaches with opportunities for water sports.

The role requires flexibility and adaptability as modern mental health services evolve in response to best practices. You will work with clients on CPA with complex needs and their families.

Strong communication and people skills are essential, along with a desire and ability to meet community service demands in a changing landscape. You will work closely with current team members to identify the best use of resources in a needs-led, patient-focused, evidence-based service.

The successful candidate will join a busy and evolving multidisciplinary team. You must demonstrate excellent clinical skills, the ability to work under pressure, and experience managing patients with high acuity and risk.

Key Responsibilities:
  1. Lead Practitioner and coordinate care for caseloads of the most complex/high-risk patients.
  2. Undertake initial assessments of individuals and their families.
  3. Complete risk assessments, create, and review care plans.
  4. Contribute to risk management and safety planning.
  5. Contribute to the Duty system.
  6. Support other clinical activities.
  7. Ensure clinical practice is evidence-based and aligned with NICE guidelines and Trust policies.
  8. Provide supervision of junior staff and trainees where appropriate.
