A healthcare organization in Rochdale seeks a qualified Nurse to join their Community Mental Health Team as a Care Coordinator. This full-time position involves managing a defined caseload and working with a supportive multidisciplinary team. Candidates should have current registration as a Nurse and experience in community mental health is desirable. Flexibility in working hours is required.
We have an exciting opportunity for a qualified Nurse to join our Community Mental Health Team as a Care Coordinator. In this role you will work alongside an experienced, knowledgeable, hardworking and very supportive multidisciplinary team. You will be supported to develop your existing skills, knowledge and experience within a Community Mental Health Team setting.
We currently have a vacancy for a full time Band 5 Community Mental Health Practitioner within our HMR Community Mental Health Team (CMHT) based at Sudden Resource Centre in East team. We are a Community Mental Health Team that works with adults aged 18 years upwards. The post is currently based within the Care Coordination function of Sudden Resource Centre Rochdale. However, you may be expected to work within any function of the three HMR CMHTs in line with the service requirements.
You will have a defined caseload and also be expected to participate on the team's duty rota. This is a full time post working core hours of Monday to Friday 08:45-16:45hrs. Some flexibility will be required with working hours due to the nature of the role and when covering the duty service.
PARIS training would be an advantage; however training will be provided if required.
