Job summary

CNTW NHS Trust is seeking a Community Practitioner to join a rural, well-established multidisciplinary team. You will assess and treat clients with complex mental health needs, collaborating with families and other professionals across organisations.

You will work with a strong emphasis on autonomous practice, leadership, and ongoing supervision, while contributing to safe, high-quality care and training opportunities within CNTW.

Qualifications

• Excellent interpersonal and communication skills.
• Evidence of post-registration training relevant to the post.
• Proven post-registration clinical experience.

Responsibilities

• Complete assessment of clients' needs and identify collaborative treatment plans.
• Deliver evidence-based psychological therapies, individually and in groups.
• Support the wider team to ensure high standards of care and safety.
