Overview
An exciting opportunity within the Early Intervention in Psychosis (EIP) service in Kensington Chelsea and Westminster. CNWL is committed to developing the Nursing workforce and offering opportunities to aspiring candidates. This post can be filled by a Band 6 RMN, Social Worker or Occupational Therapist seeking a new challenge.
Responsibilities
- Carry out initial and comprehensive assessments of health and social care needs, alone or with colleagues, and feedback to the Multi-Disciplinary Team.
- Care co-ordination for an identified group of service users, contributing to a service committed to recovery and social inclusion.
- Undertake MDT activities including on-duty and clinic rota work as appropriate within your profession.
- Be a role model to Band 5 RAD nurses and student nurses in the team.
- Provide nursing centred on an individual’s strengths to maximise autonomy and deliver NICE concordant care for First Episode Psychosis.
- Work with individuals experiencing their first episode of psychosis (ages 14 to 35) with a high prognosis for recovery.
- Educate health professionals and other services (e.g., ASC and voluntary sector) about First Episode Psychosis and the EIS role, including pre-referral advice and consultation to support appropriate triage and referrals.
- Ensure and clarify patient pathways through mental health services; undertake reviews and formulate care plans to meet identified needs.
- Conduct risk assessments and develop risk management plans per Trust policies.
- Participate in clinical meetings to discuss patient needs and promote a shared approach to care, treatment and risk management.
- Plan and implement care underpinned by best practice guidance and person-centred principles; review and evaluate care and risk management plans regularly.
- Advise on medication management, promote concordance, monitor effects and side effects in line with professional guidance.
- Facilitate physical and mental health reviews; provide information, advice and assessment to carers as required.
- Work with primary care to ensure timely referrals to secondary services and escalate more complex cases as needed.
- Work as part of the mental health practitioner team in base, clinics and service user homes as appropriate.
- Develop and maintain access to community and voluntary agencies/services for additional support and therapeutic options.
- Ensure safe discharge back to primary care when secondary services are no longer required.
- Promote and participate in joint training events within the Organisation/Borough as agreed with management.
- Encourage and facilitate involvement of service users in all aspects of service delivery.
- Participate in clinical and business meetings; create and maintain data systems including audit; supervise junior colleagues and students.
